Effective February 2010, The New York State Department of Health has enacted health and safety standards for all providers delivering Early Intervention Services. These standards can be accessed at:

The NYC Early Intervention Program is issuing a set of new and revised policies, procedures and forms as part of an updated Policy and Procedure Manual.
For your reference, enclosed in the previously released Policy and Procedure Manual is a new chapter – Chapter 6 – Service Delivery. In this chapter you will find the following new items:

6C - Obtaining Prescriptions for Authorized Services Requests for Prescriptions for Services – Sample Language
